    Canceling Appointment for ALL Suncoast Services - A note from CEO Bradley Herremans:
    I’d like to take a moment to thank all of our Patients for choosing Suncoast Community Health Centers for your care.  Our Doctors, Dentists, Pharmacists and all the other incredible providers employed by Suncoast are some of the best at what they do.  I’d like to ask a favor of each of you, please help us out with a difficult issue that we have been experiencing.  No Show Appointments.   As a community health center, access for our patients is very important.  When an appointment goes unused that is one less opportunity for somebody to see one of our providers.  So if you cannot make your appoint please call to reschedule.  That opens up that appointment for somebody else to use.  The number to call to make and to cancel an appointment is:  (813) 653-6100

    Suncoast provides services on a sliding fee scale based on schedule E (100%), D (75%), C (50%) B (25%) A ($15) standard fee according to family income and the number of family members in the household.

    Suncoast Community Health Centers, a not-for-profit organization, provides high quality, affordable health care to southern and eastern Hillsborough County and OB in Lakeland, FL. We believe that each and every community member is entitled to quality health care. No patient will be turned away because of inability to pay.


    Suncoast accepts the following forms of payment:

  • Private Pay Pricing

    Suncoast Community Health Centers discounts services for patients with incomes below 200% of the Federal Poverty Line in accordance with the established sliding fee scale.

    If you are uninsured, you may qualify for a sliding fee for your medical, dental, laboratory, and medication needs. This program is intended to provide health care for people who otherwise may not receive it. You are expected to pay the discounted rate or minimum fee for services. If you cannot pay at the time of service, please make arrangements for payments.

  • How does it work?

    1. Qualify: If you are uninsured, you may qualify for a discount from our already discounted fees.
    2. Register: Schedule an appointment today with an experienced financial counselor who will register you with the pricing that fits your budget.
  • What do you mean by income?

    Income means your paycheck before deductions, pension, retirement, or social security income, cash child support, alimony, workers compensation payments, etc. from all sources in the household. All office visits are eligible for a sliding fee.

  • What do you consider a household?

    • Wife, Husband and Children
    • Mother, Father, and Child
    • Husband and Wife
    • Mother and Child
    • Father and Child

  • If other family members live in the household, please provide documentation such as:

    • Tax Returns
    • Power of Attorney
    • Guardianship
    • Court Documentation
    • Legal Documents stating financial responsibility
    • Care Taker documentation- must state the patient's name and caretaker's name


    The following items may be used as a proof of entire household income:

    • Gross wages (paystubs showing 1 month's worth of income, 4 paystubs if paid weekly or 2 if paid biweekly) of each spouse or household income and household size
    • SSI/SSDI (Social Security Income/Social Security Disability Income) letter or bank statement showing amount being deposited on a monthly basis
    • Pension/Annuities/Alimony
    • Child Support (court order showing amount entitled to receive or notarized letter of paying parent)
    • Unemployment – letter from unemployment facility or bank statement showing amount entitled to receive
    • Worker's Compensation/Retirement/Cash Assistance
    • Tax Return Form
    • Suncoast Financial Support Statement/Self Declaration of Income form (provided at your appointment)
    • Suncoast Employment Verification form if paid cash (provided at your appointment)

  • Please be advised that sliding scales require a deposit amount at the time of each office visit during the check-in process. The amount of deposit is applied towards your share of the total charges for services rendered (including laboratory testing).
