Please bring the following documents with you at the time of your appointment.

1. Proof of identification (picture ID, driver’s license or passport)
2. Contact Information (address, phone numbers, and email)
3. Insurance Card / Proof of Insurance
4. For Self-Pay Patients / Uninsured Patients - Proof of income for each working family member [current pay stubs (2 if paid bi-weekly, 4 if paid weekly) or letter from employer on company letterhead with contact information)
5. Social Security card if you have one
6. Any Prescriptions Currently Taking
7. Immunization Card (for Pediatric appointments)
8. Guardianship Forms

You are not considered an established patient until you have seen the provider.

Suncoast provides services on a sliding fee scale based on schedule E ($120), D ($95), C ($65) B ($35) A ($25) standard fee according to family income and the number of family members in the household. Click here for more information: Good Faith Estimate
